Outagamie County Housing Authority/NAHBRS provides a Service Coordinator to be a partner, help the community with daily living tasks, to coordinate programs and services to assist residents to become self-sufficient and successful.

 

Primary Duties and Responsibilities:

• Provide residents with information and a
referral list for community agencies/services and programs.

• Assist residents with understanding bills and medical forms, filling out eligibility.

• Helps residents communicate, navigate with the ADRC and Health & Human Services for County programs.

• Help residents arrange for housekeeping, shopping, transportation, Meals on Wheels, cooking and laundry assistance and legal advocacy.

• Assist with coordinating socialization activities and educational opportunities, along with helping build informal support networks and life skills.

• Available to residents to answer questions, address concerns, and provide emotional support as needed

• Works cooperatively with the Nurse Advocate to ensure health related issues are addressed.

• Helps residents understand OCHA House Rules and Policies.
